The full name of the monument is the Memorial to the Victims of Katyn, Kharkov and Mednoye Massacres and Camp Prisoners in Kozelsk, Starobelsk and Ostashkov.
The monument was mounted in Juliusz Słowacki Park in 1999. Designed and executed by Tadeusz Tchórzewski, the memorial is now taken care of the Katyn Families of Lower Silesia.
